Description
Section titled “Description”During the update from Phoenix to Qilin, a database table was not successfully migrated Green (Phoenix) to Blue (Qilin).
The migration step for the table containing the Teams definitions failed, leaving the Qilin environment without the data required to display existing teams.
Impact
Section titled “Impact”Users who previously had teams configured saw that their teams were no longer present.
All flows continued to work during the incident — the impact was limited to the UI user experience (teams missing in the interface).
Resolution
Section titled “Resolution”The missing table has been restored, and team definitions are available again.
Follow-up
Section titled “Follow-up”- Strengthen migration validation checks to ensure all critical tables are migrated.
- Add post-migration verification for Teams data when promoting changes from Blue to Green.
