## Description

During the update from **Phoenix** to **Qilin**, a database table was not successfully migrated **Green (Phoenix)** to **Blue (Qilin)**.

## Cause

The migration step for the table containing the **Teams definitions** failed, leaving the Qilin environment without the data required to display existing teams.

## Impact

Users who previously had teams configured saw that their **teams were no longer present**.

**All flows continued to work during the incident** — the impact was limited to the **UI user experience** (teams missing in the interface).

## Resolution

The missing table has been **restored**, and team definitions are available again.

## Follow-up

- Strengthen migration validation checks to ensure all critical tables are migrated.
- Add post-migration verification for Teams data when promoting changes from Blue to Green.
