# Incident Report 18/11/2025

**Start:** `Fenrir/3.8.17` (Nov 17, 2025, 21:30 AM UTC+2:00)

**End:** `Gremlins/3.9.6` (Nov 18, 2025, 11:25 AM UTC+2:00)

**Reach:** One flow was unusable across all Mindflow tenants.

**Impact:** High – blocking

---

## Description

After the Gremlins release, a previously working flow could no longer be opened in the designer.

The error message:

```jsx
Something went wrong → Failed ot load flow snapshot
```

---

## Causes

Two issues caused the problem:

1. A bug fix for webhook responses was not backported correctly to the Fenrir release.
2. An issue occurred during our delivery process.

   Our delivery process follows a "_**blue**_/_**green**_ deployment" methodology.

   When release _**N**_ runs on the _**blue**_ account, we install version _**N+1**_ on the _**green**_ account. Once installation completes, all data from the _**blue**_ account is copied to the _**green**_ account, and users are redirected to _**green**_. Meanwhile, a migration subsystem monitors changes on the _**blue**_ account during the data copy.

   The affected flow was being modified during delivery, and the migration subsystem failed. Some modifications from the _**blue**_ account were not sent to the _**green**_ account, resulting in an unstable flow.

---

## Resolution

1. To unblock the user as quickly as possible, the flow was resynchronized between the _**blue**_ and _**green**_ accounts.
2. The bug in the migration subsystem has been identified and fixed. The fix will be deployed in the next release.
3. Additional tests will be conducted to ensure the migration subsystem handles all edge cases properly.

## Follow-up

The issue will not occur with the upcoming releases.

We sincerely apologize for this disruption. We understand the inconvenience this has caused and are committed to preventing similar issues in the future.
